Pre-Screening Educational Materials
We have developed a series of risk surveys for each disease state we target.
Distributing these materials prior to initiating a screening will help influence individuals to participate in a baseline or follow-up wellness screening. Additional materials available to you include educational pamphlets, flyers, bagstuffers, posters, press releases and e-mail releases.
Personalized Reports and Diagnostic Interpretations
Each of our recommended screening instruments generates personalized patient reports with physician ready diagnostic interpretations These reports are released directly to the patient following test unless otherwise directed by your organization. Copies of the reports can also be provided to the patient's physician.
Disease State Workshops
We have prepared Power Point slide presentations for each disease state highlighting the prevention and maintainance for each targeted disease state. We recommend that these workshops be limited to no more than 20 participants. Each workshops offers an opportunity to take part in a personalized wellness screening.
Post-Screening Evaluations
We utilize exit surveys to identify important patient trends and to evaluate demographic, gender, and ethnic relationships between test scores and the patient information collected. These surveys can be customized based on your needs and objectives.
Ongoing Consultation
We offer our patients an opportunity to participate in ongoing educational programs held at the pharmacy and other local sites. These programs are designed to improve lifestyle and patient compliance through ongoing disease management. We will "keep the physician in the loop" by providing scheduled updates and trending reports generated by the ongoing series of diagnostic testing. All componentsof the physician-pharmacist-patient triad will be met.
